Welcome to the SFHS Wellness Office! SFHS employs a full-time nurse and two Athletic Trainers to meet the health needs of our students, faculty, and staff.

Health Services FAQs

If my child has a chronic condition, such as asthma or food allergies, what should I do?
If your child has asthma and/or a food allergy, we ask parents to update their child’s online health record with that information. When you update the online health record, you are asked to upload the appropriate action plans. This ensures we have the information needed in case of an emergency.
 
Is my child allowed to carry his/her EpiPen and/or inhaler while at school?
Yes, your child can and should carry his/her own emergency medication while at school; however, a doctor’s note is required. You can access this information online in Magnus Health. Students may also keep emergency medications in the Wellness Office.
 
What if my child takes a daily medication?
If your child needs medication during the school day, a consent form must be filled out and uploaded to your child’s online health record in Magnus Health. If the medication is over the counter, we require a parent’s permission via Magnus Health. If the medication is a prescription, a physician’s signature is also required. The medication authorization form is in Magnus Health. All medication is locked in the Wellness Office. All medication must be kept in the original container from the pharmacy.
 
What medical information or forms are needed for my child to attend school?
Based on the Illinois Department of Health, the following information is needed for your child to attend school:
  • A current physical examination. This must be dated within one year from the start of the school year.
  • Dental exams are required for all incoming 9th grade students, as well as new students. This must be dated within one year of the start of the school year.
  • Vision exams are necessary only for students who have never attended an Illinois School.
  • Proof of immunizations must be up to date.
  • All student athletes must submit an annual athletic physical.  Athletic physicals are good for a period of 13 months from the date they were completed.
  • All seniors must submit proof of their second meningococcal vaccine prior to 12th grade entry.

SFHS partners with Magnus Health to facilitate communication about our health services. Magnus Health is a leading provider of electronic student health record (SHR) for independent schools. The SHR is an online solution for collecting, managing, and tracking student health information. This technology helps schools save time by streamlining communication with parents and keeping schools prepared in the event of an emergency.
